Our Specialized Community Connector service pairs each individual with a connector who learns their interests and helps them build a real presence in their neighborhood — clubs, volunteering, faith communities, classes, and friendships that last beyond scheduled hours.
What's included
- Interest and strengths discovery
- Introductions to local groups, classes, and volunteer roles
- Coaching on social and self-advocacy skills
- Support building natural, unpaid relationships
- Progress notes shared with the individual's team
Eligibility
- Enrolled in a Colorado HCBS waiver (DD or SLS)
- Authorization from your case manager
- Residing in Aurora or surrounding communities
